1. To sign up by fax with Electric Checks, download the PDF Agreement form and
- Open and fill it out online, save it and print it
or
- Open and print it, fill it out by hand
(if you need the free Adobe Reader, you may use the link to the right to download
and install it on your PC or Mac)2. Fax the completed form and a voided business check to 702-543-4843
For security reasons, we do not recommend that you email the completed PDF form to us.
3. We will verify your information and contact you with a username and password and complete instructions as soon as your enrollment is accepted.
